- How families can access free or low-cost immunizations?
- How families can submit a Certificate of Medical Exemption or a Certificate of Nonmedical Exemption?
- Circumstances under which a child who is not up to date on immunizations can be excluded from school?

In addition, schools and childcare centers are required to send information about a school¡¯s immunization rates each year. You can find immunization rates by school for Diphtheria, tetanus, and pertussis (DTap, Tdap); Hepatitis B; Polio; and Varicella on the (select the orange K-12 button and select a school from the drop-down menu).?The chart below lists MMR (Mumps, Measles, Rubella) immunization and exemption rates by school, as well as the percentage of students in the process of immunization compliance.
